UA’s IRBs are responsible for ensuring the safety and welfare of participants involved in research studies conducted at UA. The IRBs spend a great deal of time reviewing each informed consent form to make sure you are given enough information in order to make an informed decision as to whether or not you wish to participate in the study. It is the IRB’s goal to ensure the informed consent form is easy to read and understand.

You can assist us in this process by providing your feedback regarding your experience as a research study participant. The following questions are designed to let the IRB members know how to improve the informed consent form and process, please complete Research Participant Survey either online or return the hard copy to the below address. Each question is completely optional and you may choose to remain anonymous or provide your contact information at the end of the survey for follow-up purposes only. Your comments are important for improving the research participant
